Office and Policies
Accepted age group: birth to 18 years
Accepted insurance: most commercial insurance plans are welcome
What to bring on day of visit: current insurance card, valid driver's license with current address (same as that on the insurance policy)
Office services: well/sick visits, prenatal interviews, school/sports physicals, certified nutritional consultations, and acne treatment with Isolaz therapies

After-hours calls / No-show policies: after-hour phone calls to the on-call physician and missed appointments without 24-hour notice will incur a $20 and $25 fee respectively.
