We're excited you are interested in joining the Westside Pediatrics family! Please complete the following steps:
Step 1 - Call the office at (281) 578-5788 to schedule your initial patient visit and go over any questions you may have about joining the practice.
Step 2 - Once your appointment is scheduled and confirmed, please complete the forms at the following Docusign link:

It will take 20-30 minutes to complete the Docusign. Click "Finish Later" to save your progress, if you can't complete the documents in one sitting.
Please only click the link ONCE and fully complete the forms before clicking the link again (e.g. if you were going to enroll a second child). Thank you!
If you would rather physically print and complete the documents instead, click HERE to download all new patient forms in one single pdf and bring the completed forms with you on the day of your appointment. Otherwise, you can find each individual form for download at the bottom of the page.
Step 3 - For new babies, please remember to bring all hospital discharge documentation to the first visit and make sure to add the baby to your insurance provider ASAP. Most insurance companies will cover the baby for the first 30 days only, however afterward you may have to pay upfront at the time of your visit.
